Use your activity tracker to notify you when a bluetooth disconnection occurs (you've left your phone behind).

If you don't already use an activity tracker, this app will run in the background and monitor your bluetooth connection status.  

Many watchfaces have this capability, but what if you don't have such an app loaded when you walk away from your phone?  This app ensures you're notified, regardless of the current screen.
